If YES, please understand that you must submit a background check form prior to the given weekend. If your background check is not received and processed prior to the given weekend you cannot serve the weekend. Please download the Background Check form and bring it to a team meeting filled out.

If YES and you already have completed a Background Check from a previous weekend, understand that once you arrive at the campground to serve the given weekend you must complete a Background Check Affidavit Form. This form requires your dated signature. This form gives your testimony that you have not committed or been convicted of criminal acts during the time of your last background check. Please download the Background Check Affidavit Form and bring it to a team meeting.

If NO and you are under 18; you must have your custodial parent or legal guardian sign a team member Medical Release Form and Hold Harmless Form in order to serve on the given weekend? Please download the Medical Release Form and download the Hold Harmless Agreement and have both items signed by a parent or legal guardian and bring them to the team meeting.
